GYT Evaluation Toolkit
Does GYT work? How can you measure the effects of GYT?
To make it easier for you, guidelines and surveys have been developed to help evaluate
the GYT campaign at your participating health center, clinic, campus, or organization.

The survey documents are provided in a Word document so that your organization can
adapt and make changes as needed. For example, if you wish to collect more data,
questions can be added, or instructions can be included.
Each survey is paired with an Excel spreadsheet so you can automatically enter data
without creating your own spreadsheet. If your organization is unable to conduct
a survey, other potential ways that you can assess or evaluate the GYT campaign
can be found in the ”Assessment Guidelines.”

Patient Survey
This survey is designed for use in your health center/clinic. Ideally, the questionnaire
should be given while the patient is waiting to see a healthcare provider.
General Public Survey
This questionnaire is designed to target any audience and assess the general impact/awareness
of GYT's campaign messages.
